We are delighted that you have decided to join the Elfers Christian Preschool family. We strive to provide a loving, nurturing Christ centered environment where your child can learn and grow.

Please review all documents in this folder and make sure to fill them out completely. A couple of the forms require a notary. Our director can notarize those for you.

We will also need:

• Copy of your child’s birth certificate
• Form #3040 (School Physical)
• Immunization Record (Or Exemption Certificate from the Health Department)
• VPK Certificate (if enrolling in our VPK program)

Your child will need:

• Vinyl Kinder-mat of at least 1 inch thickness
• A crib sheet for their mat.
• Small blanket for nap time.
• Ice pack and lunch box.
• Extra clothes that include shirt, shorts or pants, underwear and socks.

If enrolling in free VPK only:

• Your child will only need the extra set of clothes as they will go home before lunch and nap time.

If your child is in diapers:

• Bag of diapers to stay in class. The teacher will tell you when to bring in more.
• 1 pack of baby wipes per month.
